1. How did you hear about the Ironworkers Local 167 Apprenticeship Program?

4. Do you have any Safety Training Cards? If yes, what card(s)?
5. What school experience do you have?
6. Are you aware that you must be free of all non-prescription drugs and illicit substances?

7. Do you know that all Contractors require Drug Testing?
*
8. The 167 Ironworkers jurisdiction covers a large part of Tennessee and parts
of Arkansas, Missouri and Mississippi. Are you willing to accept jobs that may be 75
miles or more from your home?

9. Do you have a Driver’s License?
*
10. Do you have reliable and dependable transportation that is yours, to get you to your assigned jobsites?
*
11. Are you willing to attend classes at the Apprenticeship Training Center at a
minimum of two nights per week, unpaid, for the duration of your 4-year apprenticeship?

12. Are you able to lift in excess of 75 pounds?
*
13. Are you afraid of heights?
*
14. Are you willing to work overtime?
*
15. Are you willing and able to work outdoors in the heat and cold?

17. I understand that any false statement or information
that I have provided on this application, or on other documents
shall be cause for denial of an oral interview or termination
of indenture, should I be selected for the program.

When reporting to our Training Center, you are asked to bring the following documents:
- Certified Birth Certificate
- High School Diploma or GED Transcripts
- Driver’s License
- Social Security Card
- DD214 (if Military Veteran)
